Banff Seminar – Registration InformationImportant Dates
Full and Daily RegistrationsTo qualify for any of the member rates, your CSHP membership must be paid through June 30, 2012. Payments by cheque are due by January 31, 2012 (late payment may result in cancellation of your registration). Full and Daily Registrations include all educational sessions, satellite symposiums, exhibits, luncheons, course materials, name badges, and registration kits.
Cancellation PolicyAn administration fee of $50 will be charged to cancel registrations from January 1 to February 24, 2012. Registration fees will not be refunded for cancellations received after February 25, 2012. To request a cancellation or to make a change to your registration, please send an email to Amanda Cuirrier at acuirrier@cshp.ca. Ways to RegisterMarch 16, 2012 - pre-meeting registration closes. After this date, please register onsite at the Banff Center. To register quickly, please complete the Printable Registration Form in advance and bring a copy with you to the conference.
