Professional Practice Conference 2010 - Registration Information

Important Dates

• December 18, 2009 - early bird registration deadline;
• January 22, 2010 - conference pre-registration closes. After this date, please register onsite at the Sheraton Centre Hotel. To register quickly, please complete the Printable Registration Form in advance and bring a copy with you to the conference.

Full & Daily Registrations

Full and daily registrations include all educational sessions, exhibits, luncheons (Mon & Tue), course materials, name badges and registration kits.

• Poster presenters will receive the early registration rate (full or daily); 
• New: Pharmacy Technicians may now register at the CAPT rate. Technicians who are members of either CSHP or CAPT qualify for the CAPT member rate;
• SPECIAL OFFER: all full non-member registrants (or one shared non-member registrant) automatically become members of CSHP for one year (see Bonus Membership Offer for all Non-Members).

Prices subject to 5% GST.

Shared (Group) Registrations

If more than one member of your staff would like to attend the conference for one or more days, why not consider the shared registration option? You purchase all 4 days of the conference in advance and assign each day to one member of the group. Up to 4 staff can be assigned to the registration with only one person attending per day. With the shared registration, you can pay as little as $180 plus GST per day!

• If all registrants in the group are CSHP members, the shared member rate applies;
• If any registrant in the group is not a CSHP member, the shared non-member rate applies;

Bonus Membership Offer for all Non-Members

All full non-member registrants (or one shared non-member registrant) automatically receive a bonus CSHP membership for the 2010/2011 membership year! Your membership will run from April 1, 2010 - June 30, 2011 – a 15-month membership! Memberships will be activated in April 2010. For shared registrants, please indicate who will receive the bonus membership on your registration form. If you wish to decline this offer, you may also do so on your registration form.

Cancellation Policy

The registration deadline is January 22, 2010. Full refunds are available prior to December 18, 2009. Refunds less a $50 cancellation fee will be provided for cancellations received from December 19-31, 2009. After December 31, 2009, no refunds will be given.
