Full & Daily RegistrationsFull and daily registrations include all educational sessions, satellite symposiums, exhibits, luncheons (Mon & Tue), course materials, name badges and registration kits.
Prices subject to Ontario provincial tax (13% HST). * Bonus Membership Offer for all Non-MembersAll full non-member registrants automatically receive a bonus CSHP membership for the 2011/2012 membership year! Your membership will run from April 1, 2011 – June 30, 2012 – a 15-month membership! Memberships will be activated in April 2011. If you wish to decline this offer, you may do so on your registration form. Cancellation PolicyRegistrations may be cancelled in writing without penalty up to December 17, 2010. An administration fee of $50 will be charged to cancel registrations from December 18 - December 31, 2010. Registration fees will not be refunded for cancellations received after January 1, 2011. To request a cancellation or to make a change to your registration, please send an email to Robyn Rockwell at rrockwell@cshp.ca. Ways to RegisterJanuary 21, 2011 – pre-meeting registration closes. After this date, please register onsite at the Sheraton Centre Hotel. To register quickly, please complete the Printable Registration Form in advance and bring a copy with you to the conference.
