Professional Practice Conference - Registration Information

Important Dates

• December 16 – early registration deadline;

• January 29 - pre-meeting registration closes. After this date, please register onsite at the Sheraton Centre Hotel. To register quickly, please complete the Printable Registration Form in advance and bring a copy with you to the conference.

Full & Daily Registrations

To qualify for any of the member rates, your CSHP membership must be paid through June 30, 2012. Payments by cheque are due by January 31, 2012 (late payment may result in cancellation of your registration).

Full and daily registrations include all educational sessions, satellite symposiums, exhibits, luncheons (Mon & Tue), course materials, name badges and registration kits.

• Poster presenters will receive the super early rate (until December 16) or the early rate (from November 1-February 8) on full or daily registrations;
• New: Pharmacy Technicians who are members of either CSHP or CAPT, may register at the Pharmacy Technician member rate;

Prices subject to Ontario harmonized sales tax (13% HST).

* Bonus Membership Offer for all Non-Members

If you have purchased a full registration at one of the non-member rates, you will automatically receive a bonus CSHP membership for the 2012/2013 membership year! Your membership will run from April 1, 2012  – June 30, 2013 – a 15-month membership! Memberships will be activated in April 2012. If you wish to decline this offer, you may do so on your registration form.  

Cancellation Policy

Registrations may be cancelled in writing without penalty up to December 31, 2011. An administration fee of $50 will be charged to cancel registrations from January 1 - 31, 2012. Registration fees will not be refunded for cancellations received after February 1, 2012. To request a cancellation or to make a change to your registration, please send an email to Amanda Cuirrier at acuirrier@cshp.ca.
