Individual Membership

The CSHP membership year runs from July 1, 2010 - June 30, 2011.

Membership fees are valid through June 30, 2011.

Member Types

Active

Pharmacists licensed or registered with a pharmacy regulatory body in Canada or persons with a professional degree in pharmacy. Only Active members hold the right to vote on Society issues.

Supporting

Persons who support the objectives of and pay fees to the Society but are not eligible for other membership categories.

Active-In-Training (postgraduate students and residents)* proof of enrollment is required

Persons who are enrolled in full or part-time postgraduate programs including residents, PharmD students, and persons doing fellowships. Proof of current enrollment is required to secure this rate (e.g., copy of student card, letter of acceptance or current transcript).

Student (undergraduate pharmacy students) * proof of enrollment is required

Persons enrolled in an undergraduate pharmacy program. Photocopy of student card is required to secure the student rate.

Joint

Two Active members residing at the same residential address. Only one person receives the regular membership mailings, although both are members. Each member pays the joint member fee (refer to fee schedule for details).

Retired

Retired members pay half the total fee for Active/Supporting. On the application or renewal form, please indicate your date of retirement.

Leave of Absence (LOA)

Designed for Active members on sabbatical, illness, or maternity/parental leave. Leave must be a minimum of 6 months in duration. Members in this category will receive full membership benefits during the year. If you are an Active member and unexpectedly leave the field in the middle of the year for a reason consistent with the LOA criteria, you may apply for LOA membership during the next membership year. Members may take advantage of the category for only 3 membership years, consecutive or not, in their career. On the application or renewal form, please indicate the type and expected duration of leave (e.g., maternity leave – February 1, 20xx to January 31, 20xx).

Active Honorary Life and Honorary Life

Individuals recognized for career contributions to hospital pharmacy (peer nomination). No fee applicable.

Membership Savings Programs

Coupon Program

Our members are our most important resource so we want to reward them for spreading the word about CSHP. If someone encouraged you to join the Society, provide their name on the 'referring member' line of your application. We will reward their efforts with a $10 coupon ($5 for referring students/residents). Similarly, when you encourage someone to become a CSHP member, we would be pleased to send you the coupon! Coupons can be used toward CSHP publications, continuing education programs, or next year's membership.

New Active/ Supporting Member Discount Program

This discount applies to members who are purchasing an Active or Supporting level membership for the first time. This includes new members as well as renewing members who have only ever held a Student or Active-in-Training level membership. If you are purchasing a Joint level membership for the first time, you may also qualify for this discount. Please note that this is not a two year membership but rather a discount program that spans two consecutive membership years. For example, in the first year of membership you will save 50% of your membership fee. In the second consecutive year of membership you will save 25% of your membership fee.

This discount does not apply to the student, Active-In-Training, Retired or Leave of Absence categories.

Membership fees include national and branch fees (if any) and all applicable taxes. Full fees are payable throughout the year.

† Members residing in Yukon Territory, Northwest Territories or Nunavut may choose to be affiliated with any branch.

* US and International members pay the national fee only and are tax exempt. Those who wish to join a provincial branch must pay the applicable branch fee. Please contact CSHP for pricing details.

**Photocopy of student card, letter of acceptance or current transcript is required to secure the Student or Active-In-Training rates.

How to Join

Join or renew now for the 2010/2011 year by completing our interactive Membership Form. Completed forms must be printed and may be returned to CSHP by mail, fax, or email.
