Program highlights: The pharmacy residency program will be based on the application of current knowledge and the search of new knowledge in actual patient care by providing evidence-based, patient-centered pharmaceutical care to hospitalized patients. The program will be designed to provide the resident with professional, clinical and administration experience and training through exposure to a broad range of activities in various areas of hospital pharmacy. Following the required rotations, the resident will choose elective rotations based on interest. Our pharmacists are a diverse group represented by B.Sc or Pharm D. graduates and/or residency-trained pharmacists who practice as Clinical Pharmacists and as Clinical Pharmacy Specialists (ICU, Infectious Disease, Internal Medicine, HIV and nephrology).

Elective rotations:

General surgery	4 weeks
Cardiology	4 weeks
Pediatrics/Neonatal ICU	4 weeks
HIV outpatient clinic	4 weeks
Nephrology	4 weeks
Oncology inpatient & outpatient	4 weeks
Geriatrics	4 weeks
Outpatient anticoagulation	4 weeks
Safe Medication practices	4 weeks

Minimum required credentials:
BScPhm, registered as a pharmacy intern or pharmacist with the Ontario College of Pharmacists.

ORGANIZATION

Hotel Dieu Grace Hospital is a 305 bed acute care community hospital. Medical services include: general medicine and surgery, cardiology, internal medicine (medical student teaching unit), neurology, nephrology, critical care and mental health.

Windsor Regional Hospital operates 687 beds between 2 sites. Medical services include: family birthing centre, pediatrics (medical student teaching unit), oncology, critical care, surgery, medicine, rehabilitation, geriatrics, children and adult mental health and complex continuing care.
