Announcing the TARGETS for Excellence

We Heard You!

The CSHP Excellence program has surveyed patients and CSHP members/supporters (November 2017) to evaluate how the 3 priority themes are being reflected in the care the hospital pharmacy team provides to patients. Based on the survey results, the Excellence program has developed realistic and aspirational targets for the performance measures of those priority themes (Patient-Centred Care, Best Practice, and Communication & Collaboration).

We are happy to reveal the targets for Excellence. Help us spread the word and share the targets for Excellence with your colleagues! Together, we can make a difference!

Here’s a sneak peek of the Excellence targets.

Excellence Targets What We Heard (2017)
>75% of patients will have had a pharmacist develop and initiate a pharmaceutical care plan (PCP) for them. 75% of pharmacy departments have evaluated the impact of medication system risk reduction strategies.
>75% of patients have pharmacists (where there are pharmacists on staff) who participate in interprofessional patient care rounds to improve their medication management.

51% participate in care rounds for 76-100% of their patients.

>75% of pharmacy departments have evaluated the impact of medication system risk reduction strategies. 68% said yes, it’s been evaluated.
