Isabel E. Stauffer Meritorious Service Award Criteria
The submission consists of an online nomination and a signature form. The signature form must be signed by three CSHP members who are nominating the individual and by the current President of the nominee's branch. By signing the form, the Branch President confirms that the branch believes the nominee has served CSHP with distinction and is deserving of this award. Following the submission of the online nomination, the signature form must be completed and returned to CSHP. If possible, a copy of the nominee’s curriculum vitae should accompany the nomination.
Nominations are screened for compliance with the criteria for this award and for the required endorsements. The submissions are then blinded and sent to the Awards Committee for review and final selection.
Nominations and signature forms will be accepted until 23:59 PDT October 7, 2018.
The following eligibility requirements apply:
- The nominee has paid CSHP membership fees for the current year
- The nominee has held CSHP membership for a minimum of 15 cumulative years
- The nominee has demonstrated ongoing contribution and commitment to CSHP, primarily at the branch and chapter levels, through participating in committee work, holding office, and other activities. The candidate may have participated in other activities that have directly benefited CSHP through the advancement of its mission, vision and goals
- The nominee is neither an Honorary Member nor a Distinguished Service Award winner
- All nominators must also have paid CSHP membership fees for the current year
The recipient is recognized during an awards ceremony at the Professional Practice Conference (PPC) and is presented with a framed certificate and a cash award of $1500.
It is suggested that you compose your submission using your normal word processor and copy/paste the information into the online nomination form. This will ensure that you have a local copy in case of computer error. Save a copy of your submission for your records before you enter it online. There are no word limits associated with the different sections of the nomination form.
It is not necessary for the candidate to be made aware of the nomination. The names of the nominees will be kept confidential throughout the selection process. Only the name of the successful candidate will be made public.
You will be asked to provide the nominee's personal information, such as name, professional degree(s), telephone number, email address, and CSHP number. You will also be asked to certify that the nominee has held CSHP membership for at least 15 cumulative years.
You will be asked to provide information about the nominee's involvement at the branch and chapter levels through participating in committee/task force work, and holding office.
You will be asked to provide information about the nominee's elected or appointed positions at the national level, such as Fellows (FCSHP) Recognition Committee (formerly Board of Fellows), Canadian Pharmacy Residency Board, CSHP Foundation, CSHP Executive Committee, or liaison appointments to other organizations.
You will be asked to summarize the impact of the nominee's activities at the branch and chapter levels.
You will be asked to describe other professional activities of the nominee that have directly benefitted CSHP through the advancement of its mission, vision, and goals.
Letters of support are optional due to the fact that the submission will be blinded prior to evaluation. If letters of support are included with the submission, please keep identifying information to a minimum. Otherwise, the letters will not be reviewed during the selection process.
