Note:
- If you wish to bring a guest to the banquet, additional dinner tickets are available for purchase at $95 + tax.
- Hotel accommodation is not included in the registration fee. Attendees must book their own rooms.

Registrations may be cancelled in writing for a $50 administration charge up to two weeks before the event. No refunds will be made within two weeks of the conference start date.
